National Healthcare Decisions Day

National Healthcare Decisions Day (NHDD) is dedicated to inspire, educate and empower the public and providers about the importance of advance care planning.

The National Healthcare Decisions Day (NHDD) Initiative is a collaborative effort of national, state and community organizations committed to ensuring that all adults with decision-making capacity in the United States have the information and opportunity to communicate and document their healthcare decisions.

NHDD New York State Coalition

The NHDD New York State (NYS) Coalition was formed in 2007 to build on the grassroots efforts started in 2001 in Rochester by the Community-wide End-of-life/Palliative Care Initiative.

In New York, we encourage families to have conversations early and regularly, as well as to complete their health care proxies.

NHDD NYS Coalition Goals Include:

• Increase conversations that lead to completion of health care proxies
• Increase awareness of the MOLST Program & eMOLST in the community
• Support statewide implementation of MOLST, Family Health Care Decisions Act (FHCDA), Palliative Care Information Act (PCIA), Palliative Care Access Act (PCAA) and appropriate legislative changes.

Members of the NHDD NYS Coalition represent and partner with statewide and regional professional associations, government, health care systems, insurers, community, faith-based and advocacy organizations. These individuals and the organizations they represent on the NHDD NYS Coalition are commended for doing an outstanding job across New York State!

Members Get Involved

Members of the NHDD NYS Coalition get involved in their organizations and communites working year round to promote advance care planning and the goals of the NHDD NYS Coalition. Some ideas for getting involved include:

• Partner with other local organizations to promote NHDD
• Partner with local retail businesses by asking them to place a promotional flyer about NHDD in checkout bags
• Partner with the state library system to encourage community libraries to set up a display highlighting the CCCC advance care planning booklet.
• Distribute advance care planning booklets and brochures at community events or health fairs
• Offer to provide a guest-speaker on health-related radio or TV programs about advance care planning and NHDD.
• Set up exhibit tables at your local churches, malls, pharmacies, grocery stores, or other locations to provide information about advance care planning, advance directives & MOLST
• Give a presentation on advance care planning & share advance care planning resources to the public at large and community organizations, such as the Rotary Club, senior centers, workplace settings, faith communities, libraries, patient or family support groups, neighborhood associations
• Sponsor a community event or “town hall’ meeting about advance care planning.
